The U.S. Embassy Bridgetown is pleased to announce that representatives from the Regional Federal Benefits Office will visit Antigua & Saint Kitts, to offer services for beneficiaries or individuals with questions about U.S. Social Security and other federal benefits.
Antigua
Location: U.S. Consular Agency, Jasmine Court, Suite #2, Friars Hill Rd., St. John’s, Antigua, Antigua & Barbuda
- Monday May 8th, 2023 – 8:00 AM to 12:00 PM and 1:00 PM to 4:00 PM
- Tuesday May 9th, 2023 – 8:00 AM to 12:00 PM and 1:00 PM to 4:00 PM
Saint Kitts
Location: St. Kitts – Nevis Chamber of Industry and Commerce, Horsford Road, Basseterre, Saint Kitts (Opposite of the West Indies Open Campus Building).
- Thursday May 11th, 2023 – 8:00 AM to 12:00 PM and 1:00 PM to 4:00 PM
- Friday May 12th, 2023 – 8:00 AM to 12:00 PM and 1:00 PM to 4:00 PM
Appointments: Walk-in Services will be available. To schedule an appointment please send an email to FBU.Santo.Domingo@ssa.gov, that includes a brief description of your inquiry, Social Security Number, address and contact information. You may also call 809-368-7011 (Monday and Thursday, 8:00am to 11:00am EST) to schedule an appointment. Please bring original documents as well as a photocopy of each document.
WHAT TO BRING FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):
Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: Birth Certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates
Social Security Number Card Application: Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.
Change of Address for Social Security: Bring your current passport.
Medicare Part B Enrollment/Cancellation: To enroll in Medicare, you should complete and sign this CMS-40Band to cancel your enrollment please complete and sign this CMS-1763.
